We greatly appreciate your time and effort to bring this person to our attention so we may honor someone in the Capital region who has shown exceptional leadership, performance in several areas of community service over many years, and for contributing to the general benefit of the Greater Concord community. The nominee does not have to be a member of the Chamber or the business community. No awards will be given posthumously. Nominations are due on Monday, September 20.

The candidate:

Your nomination must include a statement highlighting the candidate’s qualifications for the award, written by one of the nominators. Please include any additional documentation to support nomination if necessary, (e.g., information about awards received, press releases or news articles citing community involvement, candidate’s resume, etc.) using the Supporting Documentation button at the end of the submission form. Please use a personal email address that you can easily access to come back in and edit or add to the form if required (if you'd rather go the paper route, please access the printable PDF form here).
